Load from driving can cause localized stress concentrations at the necks of the elements on the metal belt in the continuous variable transmission because the presswork of the elements is not properly done, and therefore, the necks can crack and break. If the broken piece is stuck between the metal belt and the pulley, the belt may break causing the vehicle to stall.
